    @.D 15 (DAILY BLOOD REQUEST AND STATUS REPORT) (DBRSR)

   1. The RED 15 (DBRSR) will be prepared by Division Clearing Stations ad        requested  by
   the senior medical command and control headquarters.

   2. The RED 15 (DBRSR) will be submitted through the DISCOM Medical Operations Center
   to the Division Surgeon daily as of 1200 to arrive NLT 1600 the game day, (unless
   specifically contradicted by OPLAM, exercise objective, LOI). Primary means of
   transmission to the Division will be a facsimile message. Alternate transmission
   mediumg will be secure VEF telephone or messenger. Facsimile messages will be routed
   through GI/AG to the Division Surgeon.

   3.   Format as follows:


        LINE                  ITEM


        ONE ALPHA             Reporting unit and location.
        ONE BRAVO             DTO of end of report period.
        TWO ALPHA             Total number blood units on-hand by report period.
        TWO BRAVO             Total number, group and type    of units issued during
                              report period.
        TWO CHARLIE           Total number, group and type    of blood units to expire
                              during report period.
        TWO DELTA             Total number, group and type    of blood units on-hand
                              at end of report period.
        TWO ECHO              Total number, group and type    of blood units to expire
                              within next 7 days or logs.
        THREE ALP14A          Total number, group and type    of blood units requested
                              from higher echelons next 24    hours.
        THREE BRAVO           Estimate of total number, group and type of blood
                              units requested next 24 hours.
        FOUR ALPHA            Total number, group and type plasma    requested from
                              higher echelons next 24 hours.

        FIVE                  Remarks: Comment on delivery service, status of
                              storage capabilities, unit movement, excess blood,

    4. The RED 15 (DBRSRD) Report provides data      on blood/plagma units   on-hand and
    requests for blood/plasma.

    5. Unit title and 6-digit map coordinates (one alpha). Report rotation initially
    and when chan8ed; otherwise state 'SAME'.
