          SUBJECT: Accident Investigation and Reporting of Ground Accidents


          1. Reference AR 385-40; Accident Reporting and Records.

              a. -Class A accidents       (*on duty fatality or damage which exceeds,
          $500@000): United States        Army Safety Center :(USASC) has first
          option to investigate. Unit will secure accident site and notify
          this headquarters that an accident has occurred. Corps Safety
          Director will determine if USASC or unit will investigate. If the
          unit investigates, a three person.board with a field grade officer
          serving as Board President will be appointed by the MSC to
          investigate. Forward report'of investigation with chain of command
          review to this headquarters, ATTN: AFZA-SLC, within 20 days.
