CLOSTRID@
A. CLMCAL SYNDROME
ClostridiuM perfiineens is a common anaerobic bacterium associ @th
three distinct disease syndromes: (a) gas gangrene or clostridial myone
(b) enteritis necroticans (pig-bel), (c) clostridial food poisoning. Each e
syndromes has very specific requirements for delivering inomk of
12erfrinLyens to specific sites to induce disease, and it is difficult to envi:
general scenario in which the spores or vegetative organisms could be i a
biowarfare agent. There are, however, at least 12 protein toxins elaboi
and one or more of these could be produced, concentrated, and used as
weapon. Waterbome disease is conceivable, but unlikely. The best av
speculation (based on virtually no exploratory data with which to sharp4
conclusions) is that the alpha toxin would be lethal by aerosol. This is
characterized, highly toxic phospholipase C. Other toxins from the org
might be co-weaponized and enhance effectiveness. For example, the c
toxin is neurotoxic in laboratory animals.
Clinical Fean=:
The clinical picture of aerosoued C, perfrinszens alpha toxin woiu
expected to be that of a serious acute pulmonary insult. Absorbed alplu-
could produce vascular leak, hemolysis, thrombocytopenia, liver damage
Other toxins admixed could modify the event.
B. DUGNOSIS
1. Routine Laborittory Findi=
Clinical laboratory findings might include anemia (due to intravas(
hemolysis), thrombocytopenia, elevated serum transaminases, and hypox
