                                   Paralysis in the BW/CW Setting


                  The differential diagnosis must include both botulinum and nerv(

           intoxications.
                  a: Nerve agent is rapid in onset (minutes to 1-2 hrs). A rigid p         5
           develops, with parasympathetic excess (salivation, miosis, sweating, iff        XY
           defecation, and urination); central nervous system dysfunction and deat
           follow. If exposure is by aerosol or vapor, constricted pupils, rhinorri        i

           bronchoconstriction also occur.

                  b: Botu@um toxin is slower     in onset (3 hrs to several days).
           Descending paralysis (bulbar to extremities to respiratory) and parasym
           blockade (dry mouth, pupillary dilation, constipation, urinary retention,        @e
           of sweating) are characteristic. Paralysis, nausea, vomiting, and diarrh
           however, occur after exposure to either nerve agent or botu@um toxin.
           Central signs (confusion, seizure, coma) are rare after botulinum, but ci
           after nerve agent intoxication.
                  c: Anticholinergics such as atropine can, of course, cause centr2
           nervous system changes such as agitation, confusion, and hallucinations          I
           as dry mouth, dry skin, and constipation. These changes could easily ol
           the correct diagnosis in a soldier who used his injector even without exp

           to an agent.
