                      Bubonic plague should be suspected in large numbers of individuals with similar


               findings of fever, mal-dse, and tender lymphadenopathy. An epidemic of pnuemonic plague


               in its early stages could be confused with U=mia, anU=, or SEB; continued deterioration


               without stab@tion effectively rules out SEB, while gram stain of the sputum, culmm, and


               presence of the plague Fl antigen is blood specimens provide more specific evidence of


               plague.


                      3, Spmific Laboratory Diagnosis


                      Yersinia @ can be readily cultured from blood, sputum, and bubo aspirates.


               Presumptive diagnosis can be made by gram stain and (if available) immunofluorescent


               s@g. Most naturauy-=urring strains of Y, @ produce an 'Fl' antigen in-vivo,

               which can be detected in serum samples by immunoassays available in field diagnostic


               laboratories.


                      C. THERAPY


                      Untreated bubonic plague has a case-fatality rate commonly reported as around 50%;

               untreated primary septicemica and pnuemonic plague are invariably fatal. Streptomycin,

               tetracyclines, and chloramphenical are highly effective if begun early (within 8-24 hours in

               pneumonic plague). Intravenous doxycyline (200 mg initially, followed by 100 mg q 12

               hours), intramuscular streptomycin (I gm q 12 hours), or intravenous chloramphenicol (I gm
