    15. MEDICAL RECORDS AND REPORTS.

        a. Health Records.

             (1) In a combat situation, health records will normally be maintained with
    personnel records IAW AR 40-403.

             (2) Unit aid stations and clearing stations will     use Standard Form (SF) 600
    for outpatients, SF 003 for dental patients, and DD Form     1380 for Carded for Record
    Only (CRO) cases and admissions. These records will be initiated when an outpatient
    is soon for the first time in the facility. They will be retained in the facility
    until treatment is complete or interrupted; then, they will be forwarded to the
    Division GI/Adjutant General-for inclusion in the health record.

             (3) Medical facilities will maintain a lo8 of outpatient visits.

        b. Use of the FMC IAW AR 40-66.

             (1) The F.IAC will be used as a medical record by all division medical
    treatment facilities (MTF'g) in tactical situations.

             (2) The FMC will be initiated at the time of admission or when the casualty
    is first seen by an aidman. It will be completed by an officer in the medical
    treatment facility.

             (3) The  Field Medical Card (FMC) will be initiated for:

                  (a) Patients admitted to a medical treatment facility and hold past 2400
    hours on the  day of admission.

                  (b) Carded for Record Only (CRO) an follows:

                      I Wounds or injuries incurred in action (WIA).

                      2 Death of US military personnel not in patient status, excluding
    cases killed in action (HIA).

             (4) Disposition. The Field Medical Card of all patients treated in the
    clearing station will be attached to SF 600. If the patient Is returned to duty, the
    FMC and SF 600 will be forwarded by the clearing station to the CII/Aa for inclusion
    in the individuals health record. The FMC and SF 600 will accompany those clearing
tation patients evacuated outside the division area. In these cases, the clearing
    station will notify the Division Surgeon and appropriate units via R&D Report.

                   (c) Medical Status Report (MEDREP)

             I The report period for the MEDREP will be 1201-1200 hours daily. Battalion
    aid stations and medical sections will submit the MEDREP (TAB D to Appendix I to
    Annex X) to the supporting clearing station NLT 1400 hours daily.
