product is present in very limited quantities at present, and is the preparation
available from the Centers for Disease Control for treating suspected cases of
foodbome botulism. Its efficacy is inferred from mixed case-report series, but
is formally unproven outside animal studies. Use requires pre-testing for
sensitivity to horse serum (and desensitization for those allergic), and
disadvantages include rapid clearance (immune elimination, etc.) as well as the
risk of serum sickness. We would recommend this product only for very
         limited, therapeutic, use.
               A "despeciated" equine heptavalent antitoxin (vs types A, B, C, D, E, F,
and G) has been prepared by cleaving the Fc fragments from horse IgG
molecules, leaving Fab'2. This technology has been applied successfully for the
preparation of human anti-lymphocyte globulins. There is no experience in
humans with this antitoxin, but there is no reason to believe its behavior would
0 differ significantly from that of similarly prepared globulins. Once bottled and
tested, we believe that this antitoxin might offer a possible option for post-
exposure prophylaxis, but quantities and availability will be limited.
