APPENDIX 4 (Personnel Service Support) to ANNEX E (Personnel) to
OPLAN 5-90
(b) Casualty reporting begins at the unit level.
The primary source of knowledge about casualties is the
INDIVIDUAL SOLDIER.
(c) The individLtal(S) observing the casualty must
prepare the initial casualty report using DA Form 115@' twitne-,s
statement) and DA Form 1156 (casltklty feeder report), or the same
information recorded an a sheet of paper if the forms are not
available. These -Forms will be forwarded through the chain of
command to the PAC.
(d) Officers and NCOs in the chain of command must
verify, complete the form (if necessary) and report casualty
information as fast as the tactical situation permits. The
reporting process will not be delayed if the witness statement is
not available at the time the reporting process began.
(5) ...... TAACS generated report (floppy disk ind hard
copy) through the chain of command to the Bde S-1. Bde S-1 will
copy files and return floppy disk to courier. Non-TACCS users
forward hard copy by most expedient means. This Rction will take
place over all other PSS -Functions.
(6) Units will prepare letters of sympathy and
condolence is soon as possible ifter the casualty incident.
Forward the letters through command channels to the Sde S-1.
Ensure letters are signed but not dated. Letters will be sent
after notification of nc?xt-o-F-kin has been confirmed.
(7) Miss casualties. See 7th Engr Bde FSOP.
b. Personnel Information Managemc?nt
(1) Units will be prepared to deliver the field 201 file
rting PSC as
soon as a supporting PSC has been identified. Units will
coordinate with Rear Detachments to ensure that all deployed
soldiers have complete field 201 files.
(2) A bict,-Ltp COPY Of all transactions will be
maintained until a database update is completed (x.E?., P-Il/P-01
Transaction Summaries are received.)
(3) The PSC will retain copies of all actions occurring
in the -Forwird irea in the field file in order to illow
incorporation into the soldier's MPRO upon EIL-ployment.
(4) Eviluition report will continue. Delivery to HODA
level within 120 days of the closing dates.
(5) The -Forward PSC element will mail the field 201 to
the PSC rear once responsibility for the soldier is lost as a
result Of transfer, separation, etc. The +orwtrd PSC will
