(b) (U) Personnel Spot Reports (PSR) will be
submitted to 61 at COSCOM MAIN anytime a unit suffers a
critical loss which may affect their tactical operations
since the previous PDS/Spot Report was submitted. The report
will be sent by the fastest means available. As a minimum,
tho report will indicate the units(s) involved, quantity of
casualties, KIA, WIA, MIA, Non-Battle losses categorized by
(0) officers, (W) warrant officers, (E) enlisted and (T)
totals; a brief description of circumstances which caused the
casualties, and key personnel losses (Cdr/principle staff).

(c) (U) Civilian Strength Accounting.
Department of the Army Civilians (DAC) and
CONUS contractors must be accuanted for in country. Changes
to the Civilian Strength RostL-,, will be submitted to t)e (31
at COSCOM MAIN and Rear. Civil:an strength rosters will

(2) (U) Casualties.

(a) (U) Each unit will use DA Form 1156
(Casualty Feeder Report) and DA Form 1155 (Witness Statement
an Individual) to report casualties. See Enclosure 4 and
(b). (U) Abbreviated Casualty Reports and
Casualty Feeder Reports with appropriate witness statements
must be completely prepared on each casualty and forwarded in
two copies. Abbreviated casualty reports will include at a
minimum the following information +or KIA, MIA, WIA,
Captured, DNBI: Name, Rank, SSN, Unit, DMOS, Cause and
Circumstances. Provide the diagnosis and hospital location
@or WIA and DNBI, if known. DA Form 1155 (witness statement)
will only be submitted for: MIA, KIA (remains not
