U.S. control. If DA Form 1155 or 1156 is not available. any
alternate means or type of paper may be used to collect the
necessary data. Casualty reports will be forwarded
expeditiously through Battalion PAC to the PSC, and an
information copy to the CPSCOM 01 at the MAIN and REAR. Any
additional information concerning a casualty will be
submitted to the ?SC and info copy to COSCOM 01 as quickly as
posgi@le. See casualty information flow chart at enclosure 0.
Under no circumstances will units exceed 24 hours to reach
the PSC. Transmission in by mesa Iage. precedence priority or
higher. Alternate method is by telephone or courier.
(c) (U) Notification of Primary Next-of-Kin
(?KOK). PNOM will be notified telephonically by DA when
soldiers are wounded-in-action and declared very seriously
ill. Soldiers where PNOX is also in the AO are handled in
the following manner. DA Casualty would notify ARCENT
PERSCOM a death of a spouse, child or parent has occurred
which requires notification of PNOX in theater, this is
accomplished telephonically. ARCENT PERSCOM would notify the
XACOM to which the soldier is assigned for notification.
Upon notification through the chain of command. The name of
person doing notification and the date and time of
notification must be forwarded back through the casualty
chain ASAP.
(d) (U) The letter of sympathy will normally be
prepared by the commander most knowledgeable about the
casualty. The letter will be written within 24 hours after
the time of the incident or the time the initial
casualty report is submitted. The letter will be signed,
PSC is responsible to
verify the accuracy of the address. See Enclosures 7 and 8.
(e) (U) All commanders should retrain from any
type of personal notification of casualties back to COMUS
Rear Detachment. spouses, or friends until there is a
verification that NOX has been officially notified.
3. (U) ADMINISTRATION
(a) (U) Existing procedures for submitting
recommendations for awards through AG channels remain in
effect until further notice.
(b) (U) Line of Duty (LOD) Investigation.
Death or injury due to hostile action will be presumed as in
Line of Duty; however. it is recommended that injuries be
well documented (DA Form 2173 may be used to document such
injuries as well as medical treatment records). Injuries
occurring not involving hostile action will be investigated,
either informally or formally, by an investigation officer
appointed by the next higher command having Special Courts
